Застосування багатовимірних адаптивних регресійних сплайнів (MARSplines) для прогнозування ризику посттравматичного стресового розладу у військовослужбовців

Анотація

Обгрунтування. Прогнозування ризику розвитку посттравматичного стресового розладу (ПТСР), що має значні соціальні та економічні наслідки й негативно впливає на боєздатність військовослужбовців, є актуальним науково-практичним завданням. Раннє виявлення осіб із підвищеним ризиком дозволяє своєчасно впроваджувати профілактичні стратегії, зокрема інтервенції, засновані на фізичній культурі, фізичній активності, спорті та реабілітації.

Мета. Розробити та валідизувати прогностичну модель оцінювання ризику ПТСР у військовослужбовців і обґрунтувати її застосування як інструменту підтримки рішень у педагогічному плануванні програм фізичного виховання, фізичної активності та спортивно орієнтованої профілактики й реабілітації.

Матеріали та методи. У дослідженні взяли участь 4 403 військовослужбовці, з яких 89,9% — чоловіки, віком переважно 21–50 років. Вираженість посттравматичних стресових реакцій оцінювали за шкалою Mississippi Scale for Combat-Related PTSD (військова версія), що продемонструвала високу внутрішню узгодженість (Cronbach’s α = 0,892; коефіцієнт Гуттмана = 0,920). Застосовано порівняльний і гендерний аналізи, а також методи машинного навчання з використанням Multivariate Adaptive Regression Splines (MARSplines) із поетапною оптимізацією, балансуванням вибірки та оцінюванням класифікаційних показників.

Результати. Побудовано прогностичну модель ризику ПТСР на основі MARSplines із використанням 18 дихотомічних демографічних, поведінкових і службових змінних. Фінальна модель для чоловіків включала 300 базисних функцій з порядком взаємодії шість і продемонструвала чутливість 71,1%, специфічність 69,5%, загальну точність 70,3% та помірну узгодженість між прогнозованим і фактичним ризиком (κ Коена = 0,405).

Висновки. Машинно-орієнтоване прогнозування ризику ПТСР є перспективним підходом для ранньої профілактики та цільових інтервенцій у військовому середовищі. Запропонована модель може використовуватися для ідентифікації осіб групи підвищеного ризику та обґрунтування програм профілактики й реабілітації, зокрема заснованих на фізичній культурі, спорті та е-спорті, спрямованих на зниження стресу й підвищення резилієнтності військовослужбовців.
